Everyone has met a 55-year-old who moves like they are 40, and a 40-year-old who moves like they are 60. That gap is not a figure of speech — it is increasingly measurable. Your chronological age is just how many times you have circled the sun. Your biological age is how worn your systems actually are, and it can run faster or slower than the calendar depending on how you live.

The good news is that biological age is not fixed. The bad news is that the internet is flooded with people selling you expensive ways to lower it that do nothing. This guide sorts the two: what the evidence actually supports, which levers move the needle, and how to measure whether any of it is working instead of taking a supplement company's word for it.

What Biological Age Really Measures

Chronological age is one number. Biological age is an estimate of how old your body seems based on measurable signs of wear, and there are two main ways to read it.

The first is epigenetic clocks. Over your life, chemical tags called methylation marks accumulate on your DNA in patterns that track aging. One honest caveat up front: these tools are still research-grade. Different clocks can disagree, and a single reading has real measurement noise. Use them to track a trend over time, not to obsess over one result.

The One Human Trial That Moved the Needle

Most "reverse your age" claims rest on mouse studies or supplement marketing. There is, however, one rigorous human trial worth knowing.

The CALERIE study randomized 220 healthy adults to either their normal diet or a sustained roughly 25 percent reduction in calories, and followed them for two years. When researchers later analyzed the participants' epigenetic data, published in Nature Aging in 2023, the calorie-restriction group showed a 2 to 3 percent slowing in DunedinPACE — the pace-of-aging measure. That sounds small, but the authors estimated it corresponds to a 10 to 15 percent reduction in mortality risk, an effect they compared to a smoking-cessation intervention.

Two things make this result useful rather than hype. It came from a controlled trial, not a testimonial. And it slowed the pace of aging — the dynamic measure moved even when static biological-age clocks did not, which tells us that how you live changes your trajectory. The takeaway is not that everyone should slash calories by a quarter; it is that sustained metabolic improvement demonstrably slows aging, and calorie restriction is only one way to get there.

Lever 1: Fix Your Metabolic Health First

If biological age has a master switch for most people, it is metabolic health. Chronically elevated blood sugar and insulin drive a process called glycation — sugar molecules gumming up proteins throughout the body — which accelerates aging in blood vessels, skin, kidneys, and the brain. It is no accident that blood-sugar markers feed directly into blood-based age clocks.

This is the lever with the fastest, most measurable payoff. Getting fasting glucose and HbA1c into a healthy range and keeping fasting insulin low does more to slow biological aging than any supplement on the market. It is also where the calorie-restriction result likely gets much of its power — cutting refined carbohydrate and excess calories improves exactly these numbers.

Lever 2: Build Fitness and Muscle

The second lever is physical capacity, and it splits in two.

Cardiorespiratory fitness, captured by VO2 max, is one of the strongest predictors of how long you will live — stronger than most standard risk factors. Muscle mass and strength are the other half. Muscle is not just for movement — it is a metabolic organ that pulls glucose out of your blood and protects insulin sensitivity, and its loss with age tracks tightly with frailty and mortality. Preserving it is central to aging slowly, which is why muscle and longevity deserves its own attention.

Lever 3: Lower Chronic Inflammation

Aging researchers have a word for the low-grade, background inflammation that rises with age: inflammaging. Lever 4: Diet, Sleep, and the Obvious One

The remaining levers are the ones you already suspect, now with a mechanism.

Diet does double duty — it drives metabolic health and inflammation at once. What Doesn't (Yet) Work

Be a skeptic here, because this is where money gets wasted. The supplements marketed hardest for "reversing aging" — high-dose NAD boosters, various longevity blends — do not have human trial evidence showing they lower biological age. Prescription compounds discussed in longevity circles are being studied but are not established anti-aging therapies, and self-experimenting with them carries real risk. And do not fall into chasing the test rather than the drivers: paying for repeated epigenetic clocks while your blood sugar, fitness, and inflammation go unaddressed is backwards. How to Track It Honestly

You do not need an expensive clock to know whether you are aging faster or slower — most of the signal is in a standard blood panel plus your fitness. Track the drivers on their natural timelines: inflammation and triglycerides in weeks, HbA1c over about three months, fitness and muscle over one to three months. Retest every three to six months, and if you use a biological-age estimate, treat it as one more trend line rather than a scoreboard. The markers worth tracking for longevity are the same ones that feed the age clocks, which is convenient: improve them, and both your risk and your estimated age move together.
