Of all the numbers you can put on your health, VO2 max might be the most important one most people never measure. It is the maximum amount of oxygen your body can use during hard effort, and it captures something no single blood marker does: the overall capacity of your heart, lungs, blood, and muscles working as a system. That is why it turns out to be one of the best predictors of how long you will live.

Most articles about VO2 max fixate on whether your watch's estimate is accurate. This guide is about doing exactly that: the training that actually moves VO2 max, how to structure it, and how to confirm it is working in both your watch and your blood.

Why VO2 Max Predicts How Long You Live

The evidence here is genuinely hard to ignore. In 2018, Cleveland Clinic researchers published a study of more than 122,000 adults who underwent treadmill testing, tracking who lived and who died over years of follow-up. The findings were stark. The highest-fitness group had roughly an 80 percent lower risk of death than the lowest-fitness group. Low cardiorespiratory fitness carried a mortality risk comparable to or greater than traditional risk factors like smoking, diabetes, and coronary artery disease.

And there was no upper limit. Unlike some health metrics where more stops helping, fitness kept lowering mortality risk all the way up the scale — the fitter you were, the longer you tended to live, with no plateau. The practical reading of this is liberating: you do not need to become an elite athlete. The steepest part of the benefit curve is at the bottom, so simply climbing out of the low-fitness category delivers the largest single survival gain available to most people.

First, Know Your Number

You cannot improve what you do not track. A proper VO2 max test is done in a lab with a mask measuring the gases you breathe during a maximal treadmill or bike test — the gold standard. Most people, though, will use the estimate from a running watch or fitness tracker, which infers VO2 max from your heart rate at a given pace. These estimates are not perfectly accurate in absolute terms, but they are reasonably consistent, which is what matters: the trend over months is the signal, even if the exact figure is a little off.

Get a baseline, note it, and then largely ignore the day-to-day wobble. You are looking for the line to rise over eight to twelve weeks, not to jump after a single good run.

The Base: Build Your Aerobic Engine with Zone 2

Here is the counterintuitive part: most of the training that raises VO2 max is easy. Low-intensity aerobic work — the pace where you can still hold a conversation, commonly called zone 2 — builds the underlying machinery. It grows the density of mitochondria in your muscles, expands the network of capillaries feeding them, and increases the volume of blood your heart pumps per beat. That larger, more efficient engine is what a high VO2 max is built on.

The dose that works is volume: three to four sessions a week of 30 to 60 minutes at an easy, sustainable effort. It should feel almost too easy, and that is the point — the discipline is keeping it easy rather than drifting into a moderate slog that fatigues you without the benefit of either extreme. The Stimulus: High-Intensity Intervals

A big aerobic base gets you most of the way, but to lift the ceiling itself you have to spend time near it. That means high-intensity intervals — short, hard efforts that push your heart rate up toward maximum, where the adaptations specific to VO2 max are strongest.

The best-studied format is the Norwegian 4x4: after a warm-up, you go four minutes at roughly 90 percent of your maximum heart rate — hard, but sustainable for the interval — then three minutes easy to recover, and you repeat that four times. Done once or twice a week, this protocol reliably raises VO2 max in study after study. If four-minute intervals feel like too much at first, shorter, more frequent hard efforts still help; the essential ingredient is genuinely high intensity, not the exact format. The one rule: these sessions are demanding, so you earn them with recovery, not by stacking them on consecutive days.

The Formula: Polarized Training

Put the two together and you get what endurance coaches call polarized training, and it is the whole recipe in one line: keep most of your training easy and a small slice genuinely hard, with little in the mushy middle. A common ratio is roughly 80 percent easy aerobic volume and 20 percent high intensity.

In practice, for a typical week, that might look like three easy zone 2 sessions and one or two interval sessions. Progress by adding a little volume to the easy work over time and by pushing the intervals slightly — a touch faster, or one more rep — as they get easier. Do this consistently and a 10 to 15 percent improvement over a couple of months is a realistic expectation for someone starting from average fitness. The gains slow as you get fitter, but for the health benefit, the early climb is the part that matters most.

Strength Isn't Optional

VO2 max is a cardio number, but you cannot separate it cleanly from muscle. Stronger legs let you push harder in intervals and hold zone 2 with less strain, and preserving muscle as you age protects the whole physiological system that fitness depends on. Track Progress in the Watch and the Blood

The satisfying thing about training for VO2 max is how many places the improvement shows up. Watch the estimate trend upward over weeks. Watch your resting heart rate fall — a fitter heart does more per beat, so a dropping resting rate is a direct sign of progress, and the flip side, a resting rate that suddenly climbs, is worth heeding.
